    Social Worker - Registry– Harvey, IL

     

    Join the hospital trusted by Chicago’s Southland, Ingalls Memorial Hospital, in this role as a Social Worker  

    The Social Worker intervenes with patients who have complex social needs, require assistance with eligibility determination for social program and funding sources and qualify for community assistance from a variety of special funds and agencies.  In addition, offer crisis intervention to patient and families with psycho-social needs.  This role will receive referrals from individual case managers and interdisciplinary team members (including physicians, staff nurses and other members of the care team).

     

    Required Qualifications

    • Master’s Degree in Social Work, and one year of supervised social work experience in a health care setting working directly with individuals is required.
    • Current Illinois license as Social Worker. Knowledge of discharge planning, case management preferred.
    • Thorough knowledge and understanding of regulations concerning Medicare, Medicaid, Social Security Benefits, County Welfare Programs and federally funded programs designed to fit the needs of disabled and elderly.
    • Demonstration of interpersonal relationship skills which will promote communication with patients, families, physicians, nursing staff, therapists and other health care professionals.
    • Possess an analytic ability in order to find solutions to difficult or technical problems after exploring all options and alternatives available. Demonstrates awareness of community and/or governmental agencies available to meet the patient/family needs.
    • Must have good knowledge of social work issues and trends; must be competent in social work theory and practice.

    Preferred Qualifications

    • 2-5 years’ experience

    Position does require travel to all Infusion Centers (Harvey, Flossmoor and Tinley Park).
