Registered Nurse - ED Care Manager
Join the hospital trusted by Chicago’s Southland, Ingalls Memorial Hospital, in this role as an Care Manager for the Emergency department.
The Registered Nurse (RN) is responsible for coordinating and performing direct nursing care for patients, utilizing concepts of the nursing process. Serves as resource, preceptor, mentor, and leader for physicians, professional peers and other fellow employees.
Care Management facilitates the achievement of client wellness and autonomy through advocacy, assessment, planning, communication, education, resource management and service facilitation based on the individual needs of the client.
RN Care Manager will perform an integrated role coordinating clinical care facilitation, utilization management and discharge planning within the hospital.
Works collaboratively with interdisciplinary staff internal and external to the organization in the development, implementation and evaluation of the individualized plan of care and transitional care process.
Participates in quality improvement and evaluation initiatives.
Daily rounds or on-unit communication with attending physicians.

We seek a Bachelor of Science in Nursing (BSN)
2 years prior clinical care management and/or utilization management experience
Current RN license in the State of Illinois. Current CPR required. Prefer ACM or CCM certification.
Excellent interpersonal, communication and negotiation skills. Strong analytical and computer skills.
Current working knowledge of discharge planning, utilization management, case management, performance improvement and managed care.
Strong organizational and time management skills.
Flexible hours according to hospital needs. Ability to demonstrate a commitment to patient-focused care. Ability to prioritize independently and respond to multiple simultaneous requests.
Position is full time, day shift (M-F) with weekend coverage every other weekend. 12 hour shifts 9:00 AM - 9:30 PM
