    Registered Nurse-Case Manager- Home Care- Full-Time, Days 

     

    Join the hospital trusted by Chicago’s Southland, Ingalls Memorial Hospital, in this role as a Registered Nurse-Case Manager. 

     

    Under the direction of the Home Care Supervisor, the Registered Nurse plans, organizes and directs the home care of patients in the field  in conjunction with the Hospice Interdisciplinary Team. The RN provides direct patient care with an emphasis on palliative care, provides education and support to families and caregivers. This position involves after normal office hours phone triage and/or home visits as necessary for our hospice and intermittent divisions.

     

    Required Qualifications:

    • Requires RN degree from an accredited school of nursing
    • Current Illinois RN license
    • Current CPR card (American Heart Association)
    • Must have 1-2 years of Home Care experience

     

    This position will require travel to the following primary locations and may carry over to some other areas if needed:

    • Burnham
    • Calumet City
    • Dolton
    • Glenwood
    • Lansing
    • Lynwood
    • Riverdale
    • South Holland
    • Thornton
