Join UChicago Medicine, as an Advanced Practice Provider for our Pre-surgical Testing clinic at Ingalls Memorial Hospital.    As an Advanced Practice Provider, you are responsible for comprehensive direct patient care management. In this role, you will utilize advanced assessment skills and knowledge of clinical therapeutics to evaluate, diagnose and manage patients in a pre-surgical testing clinic. The APP orders and interprets appropriate diagnostic tests, therapeutic interventions and prescribes pharmacological agents as necessary. As an APP, you will maintain continuity of patient care through patient follow-up, monitoring of patient progress and modifying plan of care as necessary. The medical social worker will also be responsible for planning, implementing, and maintaining a bereavement program to meet the needs of families/caregivers following the death of a hospice patient. These services will be provided in compliance with all TJC and IDPH standards of acceptable practice and must be in adherence to all agency policies.   Job Responsibilities:   - Interviews/assesses patient and/or family regarding social, financial, and environment factors. - Obtains necessary medical, nursing and other insights from appropriate personnel to assist in determining the meeting patient needs. - Communications to other home health or hospice team members any social, emotional and/or environment problems which may affect the patient’s response to treatment. - Informs patient and/or family of community agencies and resources available to assist with patient’s needs. Demonstrates necessary skills and knowledge to provide care for patients according to unit-specific competencies as appropriate to the age of the patients served, as delineated in the Center's Scope of Care.    Essential Job Functions - Promotes the values and mission of Ingalls Memorial Hospital. - Promotes clinical excellence and service. - Acts as a patient/family advocate. - Provide comfort measures and assistance to patients and families. - Performs hourly rounds on patients and meets needs as appropriate. - Demonstrates the necessary skills and knowledge to provide care for patients according to unit-specific Scope of Care. - Understands Growth and Development and responds effectively to the unique needs of specific patient populations. - With the leadership of the staff nurse, follows the plan of care by completing assigned tasks related to direct patient care. - Assists with admissions by setting up patient rooms, orienting patients to rooms, obtaining height, weight, temperature, pulse, respiration rate, and blood pressure (in both arms when applicable), changing clothing, labeling, and storing personal care items. - Observes and follows infection control policies and procedures by infection control policy.  Required Qualifications - Certified Nursing Assistant (CNA) - Basic Life Support Certification through The American Heart Association - High school diploma or equivalent - Completed Medical Terminology Course - Past or current experience in an acute care hospital setting   Position Details - Job Type/FTE: Full-Time (0.9 FTE) - Shift: Nights - Unit/Department: Cardiac Telemetry/East 6 - CBA Code: SEIU
